Participation Agreement

By submission of this form you agree to deliver the donated goods/services, indicated above, by May 21st to the Charity Network NYC for use in the silent auction portion of the Ivy Cup Polo Match. You understand that the distribution of your donation is, from then on, under full control of the Ivy Cup Association.
